in organizations of the main secondary, general
secondary education "

1. General Provisions
1. The state service "Issuing permits for training in the form of external studies in organizations of basic secondary, general secondary education" (hereinafter - the public service).
2. The standard of public services was developed by the Ministry of Education and Science of the Republic of Kazakhstan (hereinafter - the Ministry).
3. The state service is provided by local executive bodies of the cities of Astana and Almaty, districts and cities of regional significance (hereinafter referred to as the service provider).
The acceptance of the application and the issuance of the result of the provision of public services is carried out through:
1) Non-profit joint-stock company "State Corporation" Government for Citizens "(hereinafter - the State Corporation);
2) the web portal of "electronic government" www.egov.kz (hereinafter - the portal).
2. The procedure for the provision of public services
4. Terms for the provision of public services:
1) from the date of delivery of the package of documents to the State Corporation, as well as when applying to the portal - 15 working days.
When applying to the State Corporation, the day of admission is not included in the time period for rendering the state service.
2) the maximum allowable waiting time for delivery of a package of documents to the service provider is 15 minutes;
3) the maximum allowable time for servicing by the service provider is 15 minutes.
4) the application of the service recipient for training in the form of external studies is submitted no later than December 1 of the current school year.
5. Form of the provision of public services: electronic (partially automated) and (or) paper.
6. The result of the provision of public services is an extract from the order for permission to study in the form of external studies in organizations of primary secondary, general secondary education in the form in accordance with Appendix 1 to this standard of public service (hereinafter - Appendix 1).
If the service recipient requests for the result of the provision of the public service on paper, the result of the provision of the public service is executed in electronic form in accordance with Appendix 1, printed, certified by the seal and signature of the authorized person of the service provider.
When contacting the portal, the result of the provision of public services is sent to the service recipient in the form of an electronic document certified by electronic digital signature (hereinafter - EDS) of the authorized person of the service provider.
Form for the provision of the result of the provision of public services: electronic.
7. The state service is provided free of charge to individuals (hereinafter - the service recipient).
8. Schedule:
1) the service provider - from Monday to Friday inclusive, with the exception of weekends and holidays according to the labor legislation of the Republic of Kazakhstan in accordance with the established work schedule, with a lunch break from 13.00 to 14.30 hours;
2) State Corporation - from Monday to Saturday inclusive, with the exception of Sunday and holidays according to the labor legislation of the Republic of Kazakhstan, in accordance with the established work schedule from 9:00 to 20:00, without lunch break.
The state service is provided at the location of the service provider, also through the "reservation" of the electronic queue on the portal.
3) portal - around the clock, with the exception of technical interruptions in connection with the repair work (when the service recipient contacts after the end of working hours, on weekends and holidays according to the labor legislation of the Republic of Kazakhstan, applications are received and the results of the provision of public services are provided on the next business day).
9. The list of documents required for the provision of public services when applying to the service recipient (when applying to the service recipient or legal representative with a supporting document):
to the State Corporation:
1) an application for training in the form of an external studies in the form in accordance with Appendix 2 to this standard of public service;
2) the conclusion of the medical advisory commission, form 035-1 / y, approved by order of the Acting Minister of Health of the Republic of Kazakhstan dated November 23, 2010 No. 907 "On approval of the forms of primary medical documentation of health care organizations" (registered in the Register of State Registration of Legal Acts No. 6697) - for students who are not able to attend educational organizations for health reasons;
