Standard of the state service " Provision of free and preferential food to certain categories of students and pupils in secondary schools"

Chapter 1. Generalities
      1. The state service "Provision of free and preferential food to certain categories of students and pupils in secondary schools" (hereinafter – the state service).
      2. The standard of the state service is developed by the Ministry of education and science of the Republic of Kazakhstan (hereinafter – the Ministry).
      3. The state service is provided by educational organizations, local Executive bodies of Astana and Almaty cities, districts and cities of regional significance (hereinafter – the service provider) .
      Acceptance of the application and issuance of the result of the provision of public services are carried out through:
      1) office of the service provider;
      2) web portal of "electronic government" www.egov.kz (hereinafter – the portal).
Chapter 2. The procedure for the provision of public services
      4. Terms of public service provision:
      1) from the date of delivery of documents to the service provider, as well as when contacting the portal-5 (five) working days;
      2) the maximum allowed waiting time for the delivery of documents from the service provider-15 minutes;
      3) the maximum service time allowed by the service provider is 30 minutes.
      5. Form of public service provision – electronic (partially automated) and ( or) paper.
      6. The result of rendering the state service – the certificate of free and reduced price meals in school in the form of Annex 1 to the present standard of the state service or the motivated answer about refusal in providing the state service in cases and on the grounds provided for in paragraph 10 of this standard of the state service.
      The form of providing the result of the public service is electronic and (or) paper.
      In case of the service recipient's request for the result of the provision of the state service on paper, the result of the provision of the state service is made in electronic form, printed, certified by the seal and signature of the authorized person of the service provider.
      On the portal, the result of the state service is sent and stored in the "personal account" of the service recipient in the form of an electronic document signed by an electronic digital signature (hereinafter – EDS) of the authorized person of the service provider.
      7. The state service is provided free of charge to individuals (hereinafter – the service recipient).
      8. Work schedule:
      1) service provider: from Monday to Friday inclusive, from 9.00 to 18.30 hours, with a break for lunch from 13.00 hours to 14.30 hours, except weekends and holidays, according to the labor legislation of the Republic of Kazakhstan.
      Acceptance of the application and delivery of the result of the provision of public services is carried out from 9.00 to 17.30 hours with a break for lunch from 13.00 to 14.30 hours. The public service is provided on a first-come, first-served basis, without prior appointment and expedited service;
      2) portal: around the clock, except for technical breaks in connection with the repair work (when the service recipient addresses after the end of working time, on weekends and holidays in accordance with the labor legislation of the Republic of Kazakhstan, the application is accepted and the result of the provision of public services is provided by the following working day).
      9. The list of documents required for the provision of public services at the request of the service recipient:
      to the service provider:
      1) application form in accordance with Annex 2 to this standard of public service ;
      2) identity document of the parent (required for identification);
      3) a copy of the birth certificate of the child in case of the birth of the child before August 13, 2007 or outside the Republic of Kazakhstan;
      4) a copy of the certificate of marriage or divorce (in case of marriage or divorce before 2008 or outside the Republic of Kazakhstan);
      5) a copy of the document confirming the status:
      for children from families entitled to receive state targeted social assistance - a certificate confirming the affiliation of the service recipient (family) to the recipients of state targeted social assistance provided by the local Executive bodies for the category of service recipients from families entitled to receive state targeted social assistance;
      for children from families who do not receive state targeted social assistance, in which the average per capita income is lower than the subsistence minimum-documents on income (certificate of wages of working parents or persons replacing them, on income from business and other activities, on income in the form of child support and other dependents);
      for orphans and children left without parental care, living in families - the decision of the authorized body on the approval of guardianship (guardianship), the agreement on the transfer to foster care, foster family;
      for children from families who require emergency assistance as a result of emergency - a document confirming the need for emergency assistance as a result of an emergency;
      for other categories of students and pupils, determined by the collegial body of the educational organization-the decision of the collegial body of the educational organization to provide free and preferential food to certain categories of students and pupils in secondary schools on the basis of a survey of the material and living situation of the family, as well as other necessary documents for the decision
      The documents are submitted in the originals for verification, after which the originals are returned to the service recipient.
      When receiving documents, the service provider issues a receipt to the service recipient for acceptance of the relevant documents;
      on the portal:
      1) application in the form of an electronic document, signed by the service recipient's EDS or certified by a one-time password, in the case of registration and connection of the subscriber number of the service recipient provided by the mobile operator to the portal account;
      2) electronic copy of the birth certificate of the child, in case of the birth of the child before August 13, 2007 or outside the Republic of Kazakhstan;
      3) electronic copy of the certificate of marriage or divorce, in case of marriage or divorce before 2008 or outside the Republic of Kazakhstan;
      4) electronic copy of the document confirming the status:
      for children from families entitled to receive state targeted social assistance - a certificate confirming the affiliation of the service recipient (family) to the recipients of state targeted social assistance;
      for children from families who do not receive state targeted social assistance, in which the average per capita income is lower than the subsistence minimum-documents on income (certificate of wages of working parents or persons replacing them, on income from business and other activities, on income in the form of child support and other dependents);
      for orphans and children left without parental care, living in families - the decision of the authorized body on the approval of guardianship (guardianship), the agreement on the transfer to foster care, foster family;
      for children from families who require emergency assistance as a result of emergency - a document confirming the need for emergency assistance as a result of an emergency;
      for other categories of students and pupils, determined by the collegial body of the educational organization-the decision of the collegial body of the educational organization to provide free and preferential food to certain categories of students and pupils in secondary schools on the basis of a survey of the material and living situation of the family, as well as other necessary documents for the decisionInformation on documents certifying the identity of the service recipient, the birth of a child (in the case of the birth of a child after August 13, 2007), the conclusion or dissolution of marriage (in the case of marriage or divorce after 2008), on registration as an unemployed, on the belonging of the service recipient (family) to the recipients of state targeted social assistance, a certificate of custody or guardianship of an orphan (orphans) or a child (children) left without parental care, the service provider receives from the relevant state information systems through the gateway of "electronic government".
      In the case of a request through the portal to the service recipient in the" personal account " sent the status of acceptance of the request for public service, as well as a notification indicating the date and time of receipt of the result of public services.
      In case of providing the service recipient with an incomplete package of documents according to paragraph 9 of this standard of public service and (or) documents with expired, the service provider refuses to accept the application.
      10. The grounds for refusal to provide public services are:
      1) establishing authenticity of documents submitted by a service recipient to obtain public service, and (or) data (information) contained in them;
      2) non-compliance of the service recipient and (or) submitted materials, objects, data and information necessary for the provision of public services, the requirements established by the Government of the Republic of Kazakhstan dated January 25, 2008 № 64 " on approval of the Rules of formation, direction of expenditure and accounting of funds allocated for the provision of financial and material assistance to students and pupils of state educational institutions from families entitled to receive state targeted social assistance, as well as from families, not receiving state targeted social assistance, in which the average per capita income is lower than the subsistence minimum, and orphans, children left without parental care, living in families, children from families requiring emergency assistance as a result of emergencies, and other categories of students and pupils";
      3) in respect of the service recipient, there is a court decision that has entered into legal force, on the basis of which the service recipient is deprived of a special right related to the receipt of the state service.
Chapter 3. The order of appeal of decisions, actions (inaction) of local Executive bodies of regions, the city of Republican value, the capital, areas, the cities of regional value, and also service providers and (or) their officials concerning rendering the state services
      11. Appeal of decisions, actions (inaction) of the service provider and (or) its officials on the provision of public services: the complaint is filed to the head of the service provider or the head of the relevant local Executive body of the regions, the city of Republican significance, the capital (hereinafter – akimat) at the addresses specified in paragraph 13 of this standard of public services.
      The complaint is submitted in writing by mail or on purpose through the office of the service provider or akimat, as well as through the portal.
      In the complaint of an individual, his surname, name, patronymic (if any), postal address, contact phone number.
      Confirmation of the complaint is its registration (stamp, incoming number and date) in the office of the service provider or akimat with the name and initials of the person who received the complaint, the date and place of receipt of the response to the complaint.
      When you contact through the portal, information about the appeal procedure can be obtained by calling the Unified contact center 1414, 8 800 080 7777.
      When sending a complaint through the portal, the service recipient from the" personal account " has access to information about the appeal, which is updated during the processing of the request by the service provider (marks on delivery, registration, execution, response on consideration or refusal of consideration).
      The complaint of the service recipient, received to the address of the service provider, akimat, is subject to consideration within five working days from the date of its registration. A reasoned response on the results of the complaint is sent to the recipient by mail or issued on purpose in the office of the service provider, akimat.
      In case of disagreement with the results of the public service rendered, the service recipient may file a complaint with the authorized body for the evaluation and control of the quality of public services.
      The complaint of the service recipient received by the authorized body for evaluation and control of the quality of public services shall be considered within fifteen working days from the date of its registration.
      Also, information on the procedure for appealing the actions (inaction) of the service provider and (or) its officials can be obtained by calling the Unified contact center 1414, 8 800 080 7777.
      12. In cases of disagreement with the results of the state service rendered, the service recipient has the right to apply to the court in the manner prescribed by the legislation of the Republic of Kazakhstan.
Chapter 4. Other requirements, taking into account the peculiarities of public services
      13. Addresses of places of rendering the state service are placed on:
      1) Internet resource of the Ministry: www.edu.gov.kz;
      2) portal: www.egov.kz Oh?
      14. The service recipient has the opportunity to obtain information about the procedure and status of the provision of public services in remote access through the "personal account" portal, as well as a Single contact center 1414, 8 800 080 7777.
      15. Contact phones of help services of the service provider on the provision of the state service are placed on Internet resources of the Ministry www.edu.gov.kz, service provider www.bala-kkk.kz.
