Standard of the state service "acceptance of documents and enrollment in educational institutions, regardless of departmental subordination, for training in General education programs of primary, basic secondary, General secondary education"
 
1. Generalities
      1. The state service "acceptance of documents and enrollment in educational institutions, regardless of departmental subordination, for training in General education programs of primary, basic secondary, General secondary education" (hereinafter – the state service).
      2. The standard of the state service is developed by the Ministry of education and science of the Republic of Kazakhstan (hereinafter – the Ministry).
      3. Public service is provided by organizations of primary, basic secondary and General secondary education (hereinafter – service providers).
      Acceptance of the application and issuance of the result of the provision of public services are carried out through:
      1) office of the service provider;
      2)" electronic government": www.egov.kz (further – portal). web portal
2. The procedure for the provision of public services
      4. Terms of public service provision:
      1) from the date of delivery of the package of documents to the service provider, as well as when contacting through the portal – five working days to receive a receipt;
      for enrollment in the organization of education of primary, secondary, General secondary education:
      full-time and evening form of education - no later than August 30;
      in the first class-from June 1 to August 30;
      2) maximum allowed waiting time for submission of documents – 15 minutes;
      3) maximum service time – 15 minutes.
      5. The form of public services: electronic, paper.
      6. The result of public services: the issuance of a receipt for the admission of documents and the publication of the order on admission to the organization of secondary education (primary, secondary , General secondary) at the beginning of the school year.
      Form of provision of result of rendering of the state service: electronic or paper. When you contact the service provider for the result of the provision of public services on paper, the result is made on paper.
      When you contact through the portal in the "personal account" service recipient receives a notification of enrollment in the organization of education in the form of an electronic document signed by an electronic digital signature (hereinafter – EDS) authorized person of the service provider.

      7. The state service is provided free of charge to individuals (hereinafter - the service recipient).
      8. Work schedule:
      1) service provider: from Monday to Friday inclusive, except weekends and holidays, according to the labor legislation of the Republic of Kazakhstan, in accordance with the established schedule from 9.00 to 18.30 hours with a lunch break from 13.00 to 14.30 hours.
      Acceptance of the application and delivery of the result of the provision of public services is carried out from 9.00 to 17.30 hours with a break for lunch from 13.00 to 14.30 hours.
      The public service is provided on a first-come, first-served basis, without prior appointment and expedited service;
      2) portal: around the clock, except for technical breaks in connection with the repair work.
      When the service recipient applies after the end of working hours on weekends and holidays, according to the labor legislation of the Republic of Kazakhstan, the reception of applications and the issuance of the results of the provision of public services is carried out on the next working day.

      9. The list of documents required for the provision of public services at the request of the service recipient (or his legal representative):
      to the service provider:
      1) application according to this standard; Annex 1
      2) identity document( required for identification), a copy of the birth certificate (if the child was born before 2008);
      3) on the state of health (form No. 063 / u, approved by the order of the acting Minister of health of the Republic of Kazakhstan from November 23, 2010 № 907 "on approval of forms of primary medical documentation of health organizations" (registered in the Register of state registration of regulatory legal acts under № 6697) and , approved by order of the Minister of health of the Republic of Kazakhstan dated 24 form No. 026/u-3 June 2003 No. 469 "on approval of the Instruction on filling in and maintaining the accounting form 026/u-3" child health Passport "(registered in the Register of state registration of normative legal acts under No. 2423));
      4) photos of the child size 3x4 centimeters in the amount of 2 pieces.
      For admission to training in the organization of education of children with special educational needs, the conclusion of the pedagogical, medical and psychological Commission with the consent of legal representatives is additionally submitted.
      Service recipients-foreigners and stateless persons also submit one of the following documents determining their status, with a note on registration at the place of residence:
      1) alien-a residence permit of a foreigner in the Republic of Kazakhstan;
      2) a person without citizenship – identity stateless persons;
      3) refugee-refugee certificate;
      4) asylum seeker - certificate of the asylum seeker;
      5) oralman – identity card of oralman.
      Upon delivery of documents for the provision of public services, the service recipient shall be issued a receipt for receipt of documents from the service recipient in the form in accordance with this Annex 2 to the standard of public services.
      On the portal:
      1) application of one of the parents( or other legal representatives) of the service recipient in the form of an electronic document signed by the EDS of its representative, indicating the actual place of residence of the service recipient;
      2) electronic copy of the birth certificate (if the child was born before 2008);
      3) electronic copies of documents on the state of health , approved by the order of the form № 063/from the acting Minister of health of the Republic of Kazakhstan dated November 23, 2010 № 907 "on approval of primary medical documentation of health organizations" ( registered in the Register of state registration of regulatory legal acts of the Republic of Kazakhstan under № 6697), , approved by the order of the Minister of health of the form No. 026 / u-3 of the Republic of Kazakhstan dated June 24, 2003 No. 469 " on approval of the Instruction on filling in and maintenance of the "child health passport" (registered in the Register of the form 026/u-3 of the state registration of normative legal acts under No. 2423));
      4) digital photo of a child size 3x4 cm in the amount of 2 pieces.
      Information about the identity document of the service recipient, the birth certificate of the child (if the child was born after 2008) address information service provider receives from the relevant state information systems through the gateway "e-government".
      When contacting the service recipient through the portal, a notification is sent to the" personal account " -a report on the acceptance of the request for the provision of public services in the form of an electronic document certified by the EDS.
      In cases of submission of incomplete package of documents by the service recipient according to the list provided by this paragraph of the state service standard, and (or) documents with expired service provider refuses to accept the application.

      9-1. In the case of establishing the unreliability of documents submitted by the service recipient for public services, and (or) data (information) contained in them, the service provider refuses to provide public services.
      Footnote. The standard is supplemented by paragraph 9-1 in accordance with the order of the Minister of education and science of the Republic of Kazakhstan dated 25.01.2018 № 28 (shall be enforced upon expiry of ten calendar days after the day of its first official publication). 3. The procedure for appealing decisions, actions (inactions) of local Executive bodies, the city of Republican significance and the capital, district (city of regional significance) of the service provider and (or) its officials on the provision of public services
      10. To appeal against decisions, actions( omissions) of the service provider and (or) its officials on the provision of public services, the complaint shall be submitted in writing:
      1) addressed to the head of the relevant local Executive body, the city of Republican significance and the capital, district (city of regional significance) at the addresses specified in; paragraph 12
     2) to the head of the service provider at the addresses specified in the Internet resources of local Executive bodies, the city of Republican significance and the capital, district (city of regional significance) with the name and initials of the person who received the complaint, the timing and place of receipt of the response to the complaint.
      The complaint of the service recipient on the provision of public services received by the local Executive body, the city of national importance and the capital, district (city of regional significance), the service provider, shall be considered within five working days from the date of its registration.
      In case of disagreement with the results of the public service, the service recipient may apply to the authorized body for evaluation and control of the quality of public services.
      The complaint of the service recipient received by the authorized body for evaluation and control of the quality of public services shall be considered within fifteen working days from the date of its registration.
      Information on the appeal procedure can be obtained through the unified contact center for the provision of public services.
      In the complaint of an individual, his surname, name, patronymic (if any), postal address.
      11. In case of disagreement with the results of the state service rendered, the service recipient has the right to apply to the court in accordance with the legislation of the Republic of Kazakhstan.
4. Other requirements, taking into account the peculiarities of public services, including those provided in electronic form
      12. Addresses of service providers are specified: on the Internet resources of the Ministry (www.egov.kz in the section "Public services"), local Executive bodies of the city of national importance and the capital, district (city of regional importance).
      13. Service recipient has possibility of obtaining public services in electronic form through a portal under condition of presence of a digital signature of the parents (legal representatives).
      14. Service recipient has possibility of obtaining information on the procedure and the status of rendering the state service in a mode of remote access through the unified contact center concerning rendering the state services.
      15. The contact numbers of the reference services of the service provider for the provision of public services are placed on the Internet resource of the Ministry: edu.gov.kz, Single contact center: 1414, 8800-080-7777.
